Annual Plan Rates
- $399 Adult (First Member on Plan)
- $300 Additional Adults
- $299 Per Child
The 12-month discount plan fee is due in full upon joining. This plan cannot be combined with dental insurance. Membership is effective the 1st of the month of which payment is received. Payments are due when additional services are rendered, and it is the sole responsibility of the member to maximize their benefits by arranging all the necessary appointments within the 12-month membership period. If the appointments are not used, the member will not be entitled to a refund. Renewal payment is due the beginning of the same month each year.
